You’ll need a medical document.
A Medical Document is like a prescription. It’s a form completed by a physician or nurse practitioner that authorizes your use of medical cannabis. To obtain a medical document and register with Aqualitas, follow these three steps.

Register Online
Complete the Patient Registration Form online or print a copy and return it to Aqualitas by fax, email, or mail.

Submit Medical Document
Have your physician, health care practitioner or HelloMD online clinic complete a Medical Document and submit it to our patient care centre. Once received and verified, we’ll email you a Client ID number and log in details. This can take up to 4 business days.

Go Shopping
With your Client ID and log in details, you can shop online or by phone. As a new Aqualitas patient you will receive
a 25% discount off your first order.
